This was a self-complete questionnaire, and the question wording reflects this throughout. For the opening questions, the respondent is asked to identify service receipt owing to a specific condition (e.g. due to depression). In different contexts, it is too difficult to attribute receipt of a service to a condition (e.g. for a person with dementia attending A&E due to a fall- the fall may or may not have been due to the person’s dementia). In such instances, the respondent is typically not asked to judge whether this was due to the condition or not and all receipt is considered.
