Principal investigator(s) at PSSRU: Gerald Wistow

Start year: 2015
End year: 2019

The SUSTAIN projectís overall objective is to improve integrated care for older people living at home with multiple health and social care needs. It aims to: improve established integrated care initiatives for older people living at home with multiple health and social care needs, ensuring they are patient-centred, prevention-oriented, efficient, and safe; and ensure that improvements to the integrated care initiatives are applicable and adaptable to other health systems and regions in Europe.

The project is coordinated by VU University Medical Centre (VUmc) and the Dutch National Institute for Public Health and the Environment (RIVM), and carried out by a multidisciplinary consortium of 13 partners from nine EU Member States: Austria, Belgium, Estonia, Germany, Ireland, the Netherlands, Norway, Spain, and United Kingdom.
