SCT4    four-level self-completion tool

A four-level self-completion instrument that measures current SCRQoL in community settings. It is suitable for use in surveys and can be preference-weighted.

INT4    four-level interview tool

A four-level interview instrument that measures current and expected SCRQoL in community settings. It can be used to measure the impact services have on service users’ outcomes (gain). It is suitable for face-to-face interviews and can be preference-weighted.

CH4 care home three-level mixed-methods tools

A suite of mixed-methods tools which generates current and expected SCRQoL based on observations and interviews for use in communal living settings. It can be used to measure the impact services have on service users’ outcomes (gain).

Only for use by trained users (Please note that you must be licensed to access and/or use the CH4 tools; licence is conditional on completion of a CH4 training course by the ASCOT team. If you would like to obtain a copy of this instrument, please complete our online form). It can be used to measure the outcomes of people with cognitive impairment (e.g. dementia) and communication difficulties.  It can be preference-weighted.

 A four-level self-report version for family/friend (unpaid) carers that measures current carer SCRQoL.  It is suitable for use in surveys, and is now preference-weighted.

A four-level interview version for family/friend (unpaid) carers that measures current and expected carer SCRQoL. It can be used to measure the impact services have on carers’ outcomes (gain).  It is suitable for face-to-face interviews, and is now preference-weighted.

SCT4 Easy Read    four-level self-completion Easy Read tool – for people with intellectual disabilities and/or autism

A four-level self-completion Easy Read tool (ASCOT SCT4 Easy Read) that measures current SCRQoL, designed specifically for people with intellectual disabilities and/or autism. The tool has been cognitively tested with service users and is being piloted for psychometric testing. It is suitable for use in surveys but is not preference-weighted. We currently do not licence this version for use with older adults with Dementia.

ASCOT-ER (OP)

A four-level self-completion Easy Read tool that measures current SCRQoL, designed with older people with dementia and their family/friend carers. The tool has been cognitively tested with people who use social care and will be psychometrically tested as part of future work.

ASCOT-Proxy for completion by family/friend carers or care workers on behalf of the service user.

A four-level proxy-report version that measures current SCRQoL, for completion by unpaid (family/friend) carers or care workers on behalf of people using social care services.

ASCOT-Proxy has been cognitively tested with unpaid carers and care workers reporting on behalf of someone they support, and has been psychometrically tested with family carers of people with dementia and care staff supporting older adults in care homes in England.
